What to provide

1. Employment  & Other Income slips (T4s, T4As, T5s, T3s, T5008, T4AOAS, T4P, etc.)
2. Receipts (RRSP, Dues, Donations, Childcare, Moving Expenses. etc.)
3. Investments details and carrying charges.
4. Disability certificates, death certificates and power of attorney if applicable. 
5. Business or rental income and expense information.
6. Student tuition and interest documents.
7. Changes in your primary residence, and any documentation of sales of real estate.
8. Medical expenses (See below for all details)

You can contact the pharmacy and/or your practitioner to get an annual statement.  They can print a summary off for you for all of your paramedical visits or drug purchases; Having the summary will also save time and cost as your preparer will spend less time if this is already summarized for them.

If you have a health plan, please print off an annual report of the yearly claims through your online portal. It will show you what was reimbursed and what was not.  

If you pay for a health plan through work or personal, make sure you include that detail as well. You are entitled to claim that as a deduction.

If you had to travel for medical specialists, you could claim this too. You will need to include the dates, distance and any receipts incurred while traveling such as meals and hotels.
